Today's session welcomes Hanna Stevenson and Sam Pollock from Core Assets talking about One Page profiles and EHCPs.  Core Assets Children’s Services are working on behalf of the Council for Disabled Children to provide the Independent Support Service in Cambridgeshire.  This free service offers information and support for children, young people and their families, who are involved in transferring from a Statement of SEN or Learning Difficulty Assessment (LDA) to an Education, Health and Care Plan (EHC). They can also support families requesting a new EHC and families where an EHC needs assessment has been agreed, and families who want more information around the new SEN/D reforms generally.

 As part of their free Independent Support service they work with individuals, informal groups and education settings to create One Page Profiles for children and young people with SEND as part of the EHC Plan process.

A One Page Profile is a person centred tool that captures important information about a child/young person on a single piece of paper. Trained Independent Supporters work one to one with children/young people or in small groups to create the One Page Profiles. The layout, design and contents can be adapted to suit each individual.
