Transitions Partnership Board

The Transitions Partnership Board is a multi-agency group. It is chaired by Councillor Shona Johnstone. The Board decides the Transitions Protocols which govern how young disabled people transfer to adult services.

We were commissioned by the Transitions Team to help them set up a parents forum. The forum had its first event in November 2009 when we ran a workshop on the Transitions Protocols. Parents came up with a range of suggestions on how the Protocols could be improved, and a number of actions were agreed.

Two parents have been elected to represent the forum on the Transitions Partnership Board: Lorraine Williamson represents the younger age group, and Anne Mitchell the older.

If you would like to know more about joining the Transitions Parent Carer Forum email Claire Bailey the Transitions Team Co-Manager.

If you are interested in becoming a parent representative on this Board, email Lynn or phone/text her on 07871 558850.

IMPORTANT DATE FOR YOUR DIARIES: 3 May 2012
